Mobile Unit Forms
Each person who is participating in a Mobile Unit program with LAC must have a signed Life Adventure Center Consent Form. (See Below)
Individuals under the age of 18 must have the signature of parent or legal guardian.
Please ensure that everyone in your group has a signed form even if they do not intend on participating.
